Reporting An Issue in Delivery Exception Form APP


At times, delivery issues may arise that require immediate attention. Whether it's an allergen or damaged item, an expired product, an item not on the truck, a mispicked item, or a rejected delivery, our Delivery Exception Form APP is here to help.


Here's how to report an issue using the app:


1. Open the Delivery Exception Form APP on your device.  This can be filled out on your phone or computer.


2. Fill out the form with as much information as possible.   Provide a detailed description of the issue in the space provided.


3. Be sure to include photos of the product, invoice and shipping label/label from vendor, when applicable.  If you're using your phone, you can take photos from inside the APP, it's not necessary to take photos in advance.


4. Click on the "Submit" button to send the report.


Once you have submitted the report, our team will review it and take appropriate action. We may contact you for additional information.
